Student-Athlete Pre-Participation Required Information:
**AU uses an online database for all medical record and forms. Below you will find a helpful step-by-step guide, how-to links, and FAQ’s in order to help you to complete all of the necessary forms. The physical form is available for download in the ATS system under e-files or below.
1. Go to the following website:
aurorau2.atsusers.com
2. Log in using the following information:
Athlete ID: Student ID Number (include the zero in your student id number (0198442) if applicable)
Password: Aurora25!
(FOR RETURNERS, USE YOUR PASSWORD FROM LAST YEAR)
3. Fill out all of the information on the following tabs:
a. General
b. Insurance (primary health insurance required)
c. Contacts
4. Access the Athlete Forms tab and complete the following forms:
a. Health History
b. Assumption of Risk, Release and Indemnification Agreement
c. Medical Treatment, release of information and related issues form
d. NCAA Concussion Information Waiver
e. Due Diligence Inquiry
5. Access the e-Files tab
a. Download the physical form (also available
here)
b. Upload Sickle Cell Testing Results
c. Complete the form by AUGUST 1
6. Return the completed physical to Aurora University via:

7. Make sure all of this information is completed
